Why did '007' leave Grey's Anatomy? George O'Malley's exit, explained
George O'Malley — nicknamed "007" (as in "licence to kill") after botching his first solo surgery — was one of Grey's Anatomy's five original interns. He was played by T.R. Knight from the show's 2005 premiere through the season 5 finale in 2009.
His exit was dramatic on screen and messy behind the scenes. Here's what happened on both sides.
How George dies on the show
In season 5, George surprises everyone by enlisting in the US Army as a trauma surgeon. Before he can say goodbye to his mother, he pushes a stranger out of the path of a moving bus — and is hit himself. He arrives at Seattle Grace Hospital so badly disfigured that none of his colleagues recognise him.
The reveal comes when he traces three characters into Meredith Grey's palm: 007. His old nickname. By the time his friends realise who he is, it 's too late. George dies in surgery, appearing in one final dream sequence alongside Izzie Stevens before fading out.
It remains one of Grey's Anatomy's most gut-punching exits — nearly 20 years on.

Why T.R. Knight actually left
The behind-the-scenes story is more complicated. Knight cited three factors in interviews at the time:
- Reduced screen time — George had been increasingly sidelined through season 5, with fewer storylines and less to do. Knight told TV Guide he felt his character was being neglected.
- A breakdown in communication — Knight described a loss of trust between himself and showrunner Shonda Rhimes, telling the press: "My five-year experience proved to me that I could not trust any answer that was given."
- The Isaiah Washington incident — In 2006, co-star Isaiah Washington used a homophobic slur on set in reference to Knight, who subsequently came out publicly as gay. Washington was eventually fired, but the fallout left a lasting mark on Knight's experience of the show.
Knight requested a release from his contract in late 2008. Rhimes tried to persuade him to stay. He declined.
"There just comes a time when it's so clear that moving on is the best decision," Knight said.
Did he ever come back?
He did — twice. In the season 15 episode "Flowers Grow Out of My Grave" (2018), George appeared briefly via archived footage during a Day of the Dead sequence. Then in season 17 (2020), Knight reprised the role properly in one of Meredith's COVID-19 beach hallucinations, where the two old friends reflected on everything they'd been through.
Showrunner Krista Vernoff said the return was about giving George — and the audience — the closure his abrupt death never provided. Knight, initially hesitant, agreed.
For the record: the nickname "007" first appears in the show's pilot episode, coined by Cristina Yang. It was supposed to be an insult.
By the time George scratched it into Meredith's palm four seasons later, it had become the most devastating callback in the show's history.
