The actress died in South Carolina in August, and the ruling took five weeks to arrive. Here's what the Greenville County coroner found, what the police are still working on, and how the last year of her life had already been documented in her own words.

She was found in cardiac arrest at a house in Greenville, South Carolina, on a Sunday afternoon in August. The official cause of death took another five weeks, and it did not close the case.

Hayden Panettiere died on August 16, 2026, at 36. Her 37th birthday would have been August 21. Emergency responders were called to a residence in Greenville that afternoon, ran advanced cardiac life support, and pronounced her dead at 2:32 p.m. The Greenville County Coroner's Office conducted an autopsy the next day and found no signs of trauma. Cause and manner of death were listed as pending further studies, which is where the story sat for over a month.

The ruling, on September 22

The coroner's office ruled the death an accidental overdose, caused by the toxic effects of five substances found in her system:

  • Fentanyl — the primary agent named in the finding.
  • 4-ANPP — a fentanyl precursor and manufacturing byproduct, routinely treated in forensic toxicology as a marker of illicitly produced fentanyl rather than pharmaceutical supply.
  • Alprazolam — the benzodiazepine sold as Xanax.
  • Methocarbamol — a prescription muscle relaxant.
  • Quetiapine — an antipsychotic, prescribed for bipolar disorder and as a sleep aid.

The coroner's office closed its file administratively. The Greenville Police Department did not, and said its investigation "remains active," with detectives continuing to follow leads.

What the report leaves open

An autopsy establishes what was in a body, not how it got there. The open questions are the supply chain and the circumstances, and both belong to the police file.

According to an Associated Press report obtained by the coroner, Panettiere had been released from a rehabilitation facility in Malibu, California, weeks before she died. Present at the house were her on-again, off-again boyfriend Brian Hickerson and his brother Zach. Zach made the 911 call. He said Brian found her unresponsive and administered Narcan.

Hickerson's history with Panettiere is documented in court records. He was arrested in July 2020 and pleaded no contest to two felony counts of injuring a spouse or girlfriend. On April 20, 2021, he was sentenced to 45 days in county jail, four years of formal probation, 52 domestic violence classes, $500 in restitution, and a five-year protective order.

"I am coming forward with the truth about what happened to me with the hope that my story will empower others in abusive relationships to get the help they need and deserve," Panettiere said in a statement at the time of his 2021 sentencing.

She had already written most of this down

This Is Me: A Reckoning was published by Grand Central Publishing on May 19, 2026 — three months before her death. In it she wrote about addiction beginning in her teens, postpartum depression, the domestic violence, and the 2018 decision to give custody of her daughter to Wladimir Klitschko so she could focus on recovery. She also came out as bisexual.

The career the obituaries led with

She was in commercials at eleven months old and had done more than fifty by age five, then worked on the soaps One Life to Live and Guiding Light before she was ten. Remember the Titans and Ice Princess came next. Claire Bennet on NBC's Heroes from 2006 to 2010 won her a Saturn Award. Juliette Barnes on ABC's Nashville from 2012 to 2018 brought two Golden Globe nominations. She played Kirby Reed in the Scream franchise in 2011 and again in 2023.

Her brother Jansen Panettiere died in February 2023 at 28. Her father, Skip, asked for privacy after her death.

For the record: the coroner's office answered the question it exists to answer in five weeks. The police file on how a woman weeks out of a Malibu treatment center came to have illicitly manufactured fentanyl in her bloodstream is still open.
