NCIS stands for Naval Criminal Investigative Service — a real federal law enforcement agency that investigates crimes involving the US Navy and Marine Corps. And yes, the show's original title really was "Navy NCIS: Naval Criminal Investigative Service."

Expand the acronym and you get "Navy Naval Criminal Investigative Service."

The Navy is in there twice.

What the real NCIS does

It 's a civilian-run agency that handles felonies touching the Navy and Marine Corps — homicide, espionage, terrorism, fraud. Most of its special agents are civilians rather than sailors, a detail the show gets right: Gibbs is a retired Marine working as a civilian agent. The agency's lineage runs back to the Office of Naval Intelligence, founded in 1882; the investigative arm became the Naval Investigative Service (NIS) in 1966, and NIS was renamed NCIS in 1992.

Why the first title said it twice

The show premiered on CBS on September 23, 2003, as a spinoff of JAG, launched through a two-part backdoor pilot that aired as JAG episodes that spring. CBS wanted "Navy" bolted onto the front of the title so JAG's audience — and anyone channel-surfing past — would instantly clock it as a military show. Marketing won, grammar lost, and the redundant title was mocked from day one. It lasted exactly one season. From season 2 in 2004 onward, the show has simply been NCIS.
