Hilary Duff says her most unforgettable look came in the delivery room: the outfit she wore while giving birth to her third child, she revealed in a Sunday Times interview published April 11.

Hilary Duff just told one of those delivery-room stories you don’t forget — mostly because of the outfit. In a new interview with The Sunday Times, published Saturday, April 11, the 38-year-old said the thing she’ll never forget wearing was what she had on while giving birth to her third child.

The spandex turtleneck birth (yes, really)

Duff says labor came on so fast she couldn’t peel off the spandex turtleneck she was wearing. Her water broke, there was no time for pants, and she ended up delivering at home in full mockneck glory — and she knows exactly who she looked like.

"I delivered my baby at home looking like Elizabeth Holmes."

It ’s a perfectly surreal visual, and honestly, a top-tier celebrity birth anecdote.

The family lineup, quick and clear

  • Banks Violet, born 2018 — Duff’s first child with husband Matthew Koma
  • Mae, born 2021 — the at-home turtleneck arrival, also with Koma
  • Townes, born 2024 — baby No. 4 overall, and Duff’s third with Koma
  • Luca, 14 — Duff’s son with ex Mike Comrie

Prepping Luca for the reality of childbirth

Back in 2021 on Dr. Berlin’s Informed Pregnancy Podcast, Duff talked about getting Luca ready before Mae’s home birth. She wanted him to know what was coming — not in a scary way, in a respectful, practical way. She’s big on teaching him that women are strong, what labor actually looks like, and yes, how periods work. The goal: make these conversations normal for him, because they’re normal parts of life for the women he’ll grow up around.

Life now: loud, messy, and chosen

In 2025, Duff told Us Weekly that her daily life is pretty chaotic. She called herself an overworked, very tired mom who sometimes just stares at the wall at night wondering what happened — but she’s also clear she asked for this exact life and loves it. The tough stretches pass fast, the next thing always shows up, and there’s a lot of joy baked into the madness.

Matthew Koma appreciation segment

Duff also shouted out Koma for being fully in the trenches with her. One example: when a friend visited from Toronto and Duff wanted a 7 a.m. workout, she asked if he could cover the morning chaos. He packed lunches, made breakfast, got everyone dressed, and did school drop-off. She says he’s hands-on, he’s her best friend, and their nightly ritual is getting into bed and offloading the day to each other for fun. As she put it, they’re very much in the thick of it — together.
